CDS – Options Forward understands that supporting adults with intellectual and developmental disabilities (I/DD) in the community and their own homes is crucial for their well-being. Here are some strategies and resources to consider:

Our three focus points of care help to seek and meet needs the individual.  We look to create options to help the individual move forward and into a high quality of life. 

  1. Home and Community-Based Services (HCBS): National trends have shifted from institutional care to HCBS. Our services allow individuals with I/DD to live in their communities. As a result, the indivudals we serve can live independently or with support in their own homes.
  2. Person-Centered Planning: Implementing person-centered planning processes across the lifespan is crucial. This approach ensures that individuals with I/DD and their families actively participate in decision-making about their care and suppor..

  3. Accommodations and Modifications: Regardless of functional limitations, adults with developmental disabilities can live quality lives in their own homes and communities. Accommodations may include disability services, housing modifications, and adaptive equipment.
